This session will address CDM in PCN’s (Primary Care Networks) from a process perspective and how it is different form acute care and the need for a multi disciplinary care. Topics included in the workshop include:

• The integration of the region, physician and nurses and the role of the Chronic Disease Nurse evolved from a primarily medical management focus to a client centered approach addressing social factors.
• programs which have been successfully implemented in different areas
• PCN’s in rural communities and engaging the diverse cultures such as the aboriginals population in programs
