Self Management Support for Health-Care Providers

This workshop will describe self-management support and its place in the Chronic Care Model, focusing on strategies and techniques that health care providers can use in their practice. The speaker(s) will describe current initiatives and projects that support self-management, including: The 5 A’s, the Flinders Model; Motivational Interviewing; and pilot projects investigating curriculum development and addressing practitioner skills and administrative barriers to providing self-management support.
