This workshop will provide health care providers with an overview of the Calgary Health Region’s Living Well with a Chronic Condition Program and the culturally sensitive aspect of the program that targets Calgary’s diverse populations. Bus transportation will be provided for the site visit that will allow you to see the Living Well program in action.
