Chronic Disease Management - Key Performance Indicators

This workshop will be divided into two parts. In the first part the development of Key Performance Indicators for Chronic Disease Management in the Calgary Health Region will be presented. The speaker will describe the process used to involve various stakeholders, the list of indicators that were selected, measurement methods, incorporating risk profiles into outcome measurement, the distinction between population and program-based indicators and how both can be accommodated within a single Continuous Quality Improvement (CQI) framework. In the second part of the workshop, attendees will be provided on opportunity to discuss the CQI process they have implemented for Chronic Disease Management within their jurisdiction. Participants will be encouraged to share key lessons learned in selecting indicators, measurement issues, difficulties encountered and solutions that have been implemented.
