Complex Chronic Patients

As the population continues to age, it puts an ever-increasing strain on the human and financial resources, which are required to administer care. The baby boomer generation will have a dramatic impact on the number of chronic care cases, admitted to hospitals throughout the province and the country. In fact, there are presently 16 million Canadians living with chronic conditions. According to the World Health Organization (WHO), chronic disease is the number one health care issue for the 21st century.
