Awards

A special feature of this years conference is the first ever presentation of awards for high level achievement by organizations in Canada and from around the world for chronic disease prevention and management. Candidates for these awards underwent a rigorous review selection process by a scientific panel. Seven awards will be presented at the Conference Gala Dinner.

Why are we offering awards?

The Calgary Health Region and conference sponsors are dedicated to showcasing chronic disease management and prevention excellence. We hope that these awards will encourage the promotion and diffusion of best practices.

Who is eligible for the awards?

  • All concurrent and poster presentations made at the conference are eligible to be considered for an award
  • When submitting abstracts for either concurrent sessions or posters, speakers will be asked to select a maximum of two award categories for their presentation.
  • Participation in the awards process is encouraged by the conference organizers but voluntary for all presenters.

Who will choose the winners?

The Awards Working Group is developing criteria for the awards based upon the Chronic Care Model developed at the McColl Center in Seattle
